** You MUST have your athlete’s school ID number available as it is required on each form **

You must fill out ALL forms in order to participate.

* To access the online forms hold your cursor over the “Electronic Participation Forms” tab. You will get a drop down list of the forms.

*Click on the form name and fill out the information requested.

* To sign the document click inside the signature box and hold your mouse down, this will allow you to create an “Electronic Signature”. If you make a mistake and need to start over, click on the refresh icon next to the signature box and redo your signature.

* Once you have filled out all the information on each page you will have the opportunity to print the document.

* You will receive a confirmation email once the document has been reviewed by the CISD athletic training staff or the athletic director.

* Note for physicals: Students are required to use the Pre-participation - Physical Examination Form provided. NO OTHER Physical Examination Form can be accepted. Physical exams are good for one (2) school years. You are encouraged to fill out the history portion of the form prior to printing. The physical exam / medical history form will also be available for pick up from the field house.
